Description

As a Community Nurse, you will deliver primary and community health services within homes, clinics and community settings. Working closely with multidisciplinary teams, you will support clients with a range of health needs while promoting independence, wellbeing and continuity of care.

Key Responsibilities

  • Deliver community‑based nursing care across a range of health needs
  • Conduct health assessments, care planning and follow‑up
  • Provide wound care, chronic disease management and medication support
  • Support health promotion, prevention and education initiatives
  • Liaise with multidisciplinary teams and external service providers
  • Maintain accurate clinical documentation and reporting
  • Deliver culturally safe, patient‑centred care
  • Work independently while adhering to clinical guidelines and protocols


Desirable Experience

 

  • Experience working in rural or remote communities
  • Knowledge of Aboriginal and Torres Strait Islander health contexts
  • Experience in health promotion or preventative care programs

Requirements

Current, unrestricted AHPRA registration
Registered Nurse qualification
Previous experience in community, primary health or remote nursing
Strong clinical assessment and decision‑making skills
Experience with wound care, medication management and chronic disease care
Ability to work autonomously and manage time effectively
Strong communication and interpersonal skills
Commitment to culturally safe and person‑centred care
Current Police Clearance or willingness to obtain one
